4. If applicable: List of any prescription medications and supplements you may be taking, including dosages

5. If applicable: Pathology & radiology reports (unless you have already posted them to the practice)

Please note;

• Obstetrics patients will be asked if they have received the Antenatal email and if they have had recent blood tests and an ultrasound since making their initial appointment.

• If we are running behind schedule or there is an emergency patient the staff will endeavour to advise you of the delay
